You must secure a Notarized Affidavit of Loss and a clearance from the PNP-Highway Patrol Group (HPG). Submit these documents to the LTO branch where your vehicle is registered to apply for a duplicate plate. The LTO will give you an "Authorization to Use an Improvised Plate" so you can drive legally while waiting for the replacement.
