If you lose your OR/CR, immediately report it to the Land Transportation Office (LTO). You will need to file a notarized affidavit of loss and submit it along with a copy of your valid ID and the vehicle’s plate number. The LTO will process your request to issue a duplicate for the lost OR/CR. Be sure to check if the vehicle has any encumbrances or holds before requesting a new certificate.
